Put SIN on medical reports?

Do I put my SIN on EVERY page including the medical reports and doctors form (the doctor didn’t write my SIN)?

Preferably yes. Unless you can persuade your doctor to write your SIN on 200 pages, my guess, it shouldn’t be a problem to send those without SIN.

Just put the SIN on the CPP medical report =your actual medical records and reports-you should NOT mark up with anything

Ok, I will just write my SIN on the CPP medical form.
