I am going to have my doctor fill out the medical report rather than giving consent for my insurer to send my medical information as I think that’s better in my case.
There are 10 pages to the medical report for the CPP-D application. I know I have to fill out pages 1 and 2, and then my doctor has to fill out p3-10 and then mail it in. On p3 of the report, it says “Do not remove any pages from this medical report. You must return all 10 pages of this report.”
So if having your doctor fill it out, do I print all 10 pages, then fill out p1-2 myself and then take the set of documents (p1-10) to the doctor? Then once the doctor has filled out p3-10, the doctor mails the entire set of documents (p1-10) to Service Canada?
I know it can’t be submitted electronically, but I want to make sure this is how I should go about it, as this seems such an old-fashioned way lol
Just an update: Thanks to a helpful member of this forum, I have realized I was working on an old 2024 version of the CPP-D application form, which states applications have to be mailed to Service Canada, instead of the newest application form and medical report which were updated in Feb 2025, and can be submitted electronically. It seems you can also upload additional documents online, so I am hoping I can upload my entire medical file from my doctor that way, rather than mailing in hundreds of pages.
I recommend most people send in the long-term disability medical file as well. You will eventually need to give this anyways if your claim gets denied and you need to appeal to the Tribunal. The tribunal judges expect to see that type of file and will assume the worst if it isn’t there. Good luck with your application!

@David_Brannen Thank you for your input. That is very helpful. I don’t want my insurer to know yet that I am applying for CPP-D (and they haven’t asked me to do so yet), so I will submit my LTD claim file with my CPP-D application by myself for now (I recently obtained a copy of my complete claim file from my insurer at my 2yr mark), rather than giving consent to Service Canada to contact my insurer for my file. I guess the way to think of it is to send in everything !

David I must say that your guidance through your forum for all those people like us in need and has been struggling in our daily lives with one or the other medical condition and illness is already way too much stressful and by reading through this forum and watch your YouTube videos is a blessing to all those disabled out their.
I am very confused as I have read on your CPP D application guide where somewhere it mentions that I have to mail my own CPP D application form to service Canada via express Canada post but my question is I have recently got the medical forms started working on application like end of November- December 2025. The medical forms for my doctor to filled out does my doctor will need to send this directly to service canada via express post or something or my doctor can hand over the medical forms application report those 10 pages of application to me and I can send it all package including my own CPP D application form, Doctor medical report and cover letter and yes as you suggested request a copy of LTD file from insurer and send all in same day in one package and send through Canada post ?
Please advise I am very confused, anxious and overwhelmed with all this forms already. Looking forward to hear directly from you David.
You have to submit your CPP disability application online, mail it to Service Canada or drop it off at a Service Canada location. I mailed my application as a package (I had hundreds of pages) to Service Canada with Canada Post. With my application, I included a cover letter, narrative letter and my complete LTD claim file (though you can also just give consent to Service Canada to request information directly from your insurer. This is in Section H of the application).
Regarding the medical report:
You fill out p1-2 and take all 10 pages (p1-10) to your doctor. I would ask your doctor politely if you could review the forms after your doctor has filled them out, before they are submitted to Service Canada. That way, if there are any mistakes or inaccuracies, you can ask your doctor to correct them before they are submitted. Once the forms are completed, your doctor can mail them directly to Service Canada, or he/she can give you a copy (the doctor will probably keep the original for your medical chart) which you can then mail to Service Canada yourself. If your doctor is reliable, you can have him/her mail it directly to Service Canada (I think this is the most common method), but if your doctor or his/her clinic is a bit disorganized, you may want to mail it yourself.
I wanted to mail my medical report to Service Canada by myself, so that I knew exactly when it was mailed out. My doctor notified me when it was ready and I went to the clinic to pick it up. The forms were filled out perfectly (based on a draft I had provided) so there were no changes required. I then mailed the medical report to Service Canada. I received a tracking number from Canada Post showing it was delivered the next day.
With the medical report, I also included my complete medical file with an index that I added, and specialist reports. I included a cover letter for this package as well.
So in total, I sent in 2 packages. The first was my application package, then a couple of months later the package with my medical report. My CPP-D was recently approved.
It’s great Sunrise how thorough you are, I am only at close to 1 year mark after my slip and fall accident and I am already working to apply for CPP D as I know not just it has aggravated my past symptoms but it made things to much terrible for me to work again as we all know it’s just not the physical symptoms like pain and such but many much more things comes along with it such as mental issues and personal life style changes and not to over do anything. Do you recommend that I should wait or just send in the forms now. Also can I send my application form and medical report from Doctor all in at the same time in one package with Canada past or separate ?
I would send in the CPP-D application as soon as you meet the criteria. To be eligible for CPP-D, you have to have a severe and prolonged disability that prevents you from doing any substantially gainful work. “Substantially gainful work” means that you are unable to earn more than $20,971.45 (for 2026). Even if you have mental health issues and pain, if you are able to do a part-time sedentary job that pays more than $20,971.45, you do not qualify for CPP-D in 2026. Your medical records should show that you have done treatments to address all your symptoms, but that the treatments failed or only partially helpful.
Bottom line: as long as you meet the criteria, I would apply for CPP-D.
