Doctor Closing Practice

Hi Guys,

I am on LTD and the insurance has been requesting a generic “Physicians Statement” form filled out once a year by my doctor. My doctor is closing his practice. I cannot find a new doctor. I have been calling everywhere- I am on 3 waiting lists but it may take awhile. What are options if my insurer requests a “Physicians Statement” form to be filled out and I don’t have a doctor? Thanks so much!

You could look for a private “nurse practitioner led clinic” that offers primary health care. There is a fee but wait time is much less. They are popping up in Canada to fill the gap in primary health care. Some specialize in certain areas, such as mental health, cancer etc.
