I’ve been told my the insurance company that benefits will be terminated as of my COD date next year. Should I have completed the self- declaration paperwork? Is it too late to do so now? Or should I just wait until they request medical from my doctor?
Not sure what “self declaration papers work” is but you need to set up a consultation with Resolute legal (or some other LTD lawyer) if you believe you qualify for benefits beyond your COD.
Are there forms that you need to complete, at the insurance company’s request?
For example, on a bi-annual basis, I need to fill out a self-statement about my daily activities of living and capabilities/limitations.
If you have missed a deadline, I would recommend sending a written communication (email or registered post) requesting an extension for 30 days while you prepare a response to request for the information/ self-declaration.at the same time apply for consult with David’s team @resolute

I didn’t have to fill out anything, they just reviewed and said cod was approved. I did get a letter saying they couldn’t determine if I qualified for cod about a year beforehand
But it didn’t really mean anything, just that at that time, there was no determination. Then at the 2yrs they approved it.
Does it say your losing your benefits then amd they’ve already determined you don’t fit the cod, or just a general update?
