Applying for DTC - Need Guidance and Tips

I am very thankful I found this forum.

I have been on disability since June 2025 - initially 6 weeks of STD and since September 2025 on LTD through my employer’s group insurance policy.

So far, I have been grappling with all the paperwork & case manager follow ups on my own, until a dear friend recommended this forum.

I have been browsing all the sections and threads for the last 2 weeks, and now posting my situation.

I am getting ready to apply for the DTC.

My disability is a combination of mental health issues (severe anxiety and depression) and speech issues (paralyzed vocal cord impeding speech).

Therefore, I believe I qualify under the “Cumulative Effect” criteria.

My physician is super nice and supportive, and has agreed to fill out the DTC form for me.

However, he does not have any prior experience of filling out DTC.

I am also quite unfamiliar with this process and have never come across these types of issues for any friend or family member.

Can anyone please provide any guidance or tips for completing the forms?

Esp. when it comes to mental health issues (severe anxiety and depression).

I am struggling with debilitating ennui and it is taking me days and sometimes weeks to complete simple forms and formalities.

I am sorry but this process is quite intimidating for me.

Any guidance or tips anyone can provide will help me and my physician.

I am using DCC to prepare my application. We used them years ago with my Mom and they were so great! No fee’s upfront at all. My application is also “Cummulative”. They are very professional and I know there are alot of fly by nite places offering this service, but, DCC ( Disability Credit Consultants) is not like that. Hope that helps!

I applied and was approved for the DTC for mental health diagnosis, all 10 years, and I have helped multiple others get approval since mine. My GP was very open with me and we kind of worked on it together, but ultimately what they put down on their end is what matters.

Some things that I feel (I can’t know for sure) were helpful, were referencing the severity and frequency of the disabling symptoms. There is a chart with a scale, and my understanding is that if the bulk of the symptoms or issues listed are not selected by your GP as being moderate or severe / most or all of the time then you wouldn’t be approved. I’m uncertain if this has to be reiterated in paragraph form in the appropriate sections the GP fills out but I feel like it’s possible as companies that do ADHD assessments and submit the DTC on your behalf send you questionnaires. I’ve helped friends and partners fill theirs out after the were declined and once the “wording” changed they were approved on appeal. Sadly like a lot of things in the government system the wording matters on applications and if your doctor doesn’t state things quite right it leads to denial.

Secondly, as per my doctors advice, they almost always come back looking for more information/proof as the initial form does not give them much room to elaborate on your issues. Have that ready to go in excess. Notes from any medical professionals you’ve seen related to your condition for example. I sent in printouts from my pharmacy showing the first time I was prescribed a medication for depression over 10 years ago. I believe all of that helped as I was approved on the first try with no pushback or request for more info.
