My GP completed Part B of my DTC renewal, but several mental function ratings don’t reflect my actual functioning—they don’t even align with the older intake letter he used. There is also another letter from the time of my diagnosis that was not used. His notes section is very sparse, and the listed conditions/diagnoses are inaccurate as they come from a pre-diagnosis note. I provided a functional report, but only the physical section was incorporated (I also have a physical disability).
I’m preparing a clarification letter, an updated functioning report, and supporting documents, including relevant pages from my PWD medical report. I’m not asking him to copy the PWD wording—just to align the DTC form with what’s already documented.
Should I bring this to an upcoming appointment or email it ahead of time?
