If it was a work injury, I guess you would make a WSIB claim (Ontario).
I would guess if the problem is mental and it prevents you from working now, you would qualify for LTD.
I guess if the problem is a mental condition that can’t be treated, you would qualify for LTD.
You need support of your doctor and medical records/visits showing you tried to work.
Ya, I think you would qualify if you have doctor’s support.
Specific injuries or diagnosis do not qualify people for long-term disability, the issue will be whether the condition or injury impairs you to the extend that you cannot perform your own occupation or any other occupation that would be suitable for you, given you age, education, prior earnings etc.

what would they ask for exactly
You would have to prove that you are incapable of working and earning gainful amounts(aprox 14K a year)
We’re talking LTD, though, right?
The insurance company wants to know each function at work you do and why your disability prevents you from doing it.
And you need medical reports to support the disability.
