For the people who have passed the change of definition and now get their yearly form for them and your doctor to fill out. Do they ask you which doctor to send the form? I have a GP and psychiatrist so wondering which one they will send it?
They send me the form in the mail and then I take it to whatever doctor I want. It’s literally a 2 page document, all I do is fill out a few questions and then my doctor signs the back and confirms I am still disabled and nothing has changed. Once he signs it, I just scan it and e-mail it to them. I don’t even bother to send it via postal mail anymore.
