How to Handle Inaccuracies in Physician Report

I completed an assessment with a psychiatrist for my family doc so he could get medication guidance. In that appointment they took down my treatment history.

I saw a copy of the report after it was also submitted to insurance. There are a couple factual errors in it about my past history. Example - saying a treatment date started in one year, when it actually started the year before. Outside of timeline errors like that the report is accurate. I’m not sure if they misheard me or I misunderstood the question to cause the couple errors

Am I overthinking that I need to proactively let insurance know it’s wrong, or something that’s fine to leave unless it’s brought up?

If I should correct it, what’s the best way to approach it with my case manager? Thanks!

I’m not sure what the right answer is, but my opinion is that they won’t bother looking into it to that extent. In my experience, their main concern is current and future treatments, limitations, and prognosis. If you want to correct it, maybe let them know during or close to your next update with them, as that is when they will already be reviewing your file.
