# Medical reports/authorization forms-opinions?

For rehabilitation providers, general releases or IME release forms is it ok to write:  
this  
: signature is only valid on the condition that  
I receive full and complete copies of all  
medical reports, written or verbal, and that  
they are sent to my home address at the  
same time they are forwarded by this  
doctor/provider

Or would that be looked at poorly?

There is no doubt the insurance companies will look poorly on it, but I think a judge would think it very reasonable that the you get copies of any reports prepared by IME doctors or rehabilition clinics. I think you should add that you would be willing to pay the reasonable cost of copying and mailing the reports. Also, I would remove the part about verbal as that may make the whole thing impractical. If you focus only on written reports then it will be more reasonable.
